The Benefits Outreach Program helps those eligible apply for Medicaid and SNAP (formerly Food Stamps). We partner with other community agencies to determine who is in need of health insurance or food assistance, and we assist them with the application process from start to finish, ensuring that all requirements are met.
SNAP assistance is available at our Customer Connection Center, located at 112 East Liberty Street, from 8:30 a.m. to 3:00 p.m. Monday through Friday. Appointments are not needed.
Please Bring With You:
- Picture ID
- Social Security Card/Printout
- Birth Certificate
- Income Verification
- Household Verification (lease, rent receipt, utility bill)
- Verification of Resources (bank statements if applicable. Anyone receiving Social Security, MUST verify resources.)
- Pregnancy Verification if applicable
